Students and teachers, this is one final reminder about the surveys that you were given earlier this week.

Students, if your parents have completed and signed this survey already, please turn it in to your second period teacher now. If you need another copy of the survey, please ask your second period teacher.

Students please take this survey home and give it to your parents this evening. Ask your parents to complete this survey, sign it, and give it back to you so that you can give it to your second period teacher tomorrow.

This survey is part of a grant program that will help not only Forbush High School, but the entire Forbush and East Bend area.

Students, please return these surveys to your second period teacher TOMORROW. Thanks again for your help with this project.
